The square design (although made for an oval surface) would be much more useable if the hole were smaller. I am a T-12 paraplegic and therefore do not sit straight down on the gel cushion. My transfer is a sliding motion onto the seat and a rotation motion once after the transfer. The Velcro adhesive attachment on the cushion is not secure enough to hold the cushion in place. When the gel shifts- the hard surface of the toilet seat is exposed. This defeats the purpose of the gel cushion because you are now sitting on a portion of the hard toilet seat. If the hole were smaller it would correct this problem to some degree. I would not recommend this cushion for anyone who is not sitting straight down on this cushion and is not able to stand straight up.
